Pack of 12 econo piezo's from Gitty ( see above links) .50 each thats $5.99 a bag. I got em, they sound great built into my bridges. And no case to pull apart.

Is there a specific type of piezo that works best?
Some are "buzzers", others are simply thin disc-like items. Which one is better?
Also, is there a good place to get the wiring and input jacks?

You could go to a dollar store and pay a dollar for a single piezo transducer encased in a ton of plastic that you then have to smash apart to get to the honey ... or you could go to Ben and get 'em for 50 cents each (+ minimal s&h).
