Pedal (alternate) the base notes like this:

d----------
A---0---0--
D-0---0----

Play a few times, then slide up to the 5th fret
   \
d------------
A-----5---5--
D---5---5----
Then slide down to the 3rd fret
        /
d-------------
A------3---3--
D----3---3----

Now add the high note in there:
         \       /
d---------5-------3-------0-
A---0---0---5---5---3---3---
D-0---0---5---5---3---3---0-

rinse and repeat!
