Thanks for the response! I had already removed the piezo element from the plastic housing, then I had glued it to the underside of the soundboard. I soldered one wire to one side of the jack, one wire to the other side...there was no ground wire though. I thought maybe the humming was from a grounding issue--but I have no idea how to ground this pick up.
Besides the hum, it sounds good...

Take the plastic housing off and hot glue the piezo element directly on the wood, then cover the element with more hot glue.
