I don't have any pics of my license plate guitar with the plate off, but all I did was build a frame out of 1x2 oak a little bigger than the plate itself. Then notched the top side for the neck to go thru. I also installed two 27mm piezos one mounted to neck under bridge and one mounted directly to the plate up high. It's important to make sure the plate rests on top of the thru part of the neck or it wil be buzzy. I hope this helps.
Regarding your piezo question, tape and glue both work. Putting the piezo between two thin pieces off wood or between strips of foam or leather cuts down on the noise they will pick up, as the are VERY sensitive. Check out the forum.
