Easy way to make one would be to find a cigar box with a graphic you don't care about- wood or paper covered, then go to google and find a ouija graphic that catches your eye (I just did a google image search and found a ton of amazing ones, black and white and color, modern and retro) load it into something like photoshop or one of the free photo manipulation programs, and size it and layout how you want it on your box. Then I would go to kinkos or wherever and get a good quality print of it, preferably with a laser print- something that won't run when wet. Then get a bottle of Modge Podge, and glue it on with a thin coat, and build up multiple layers of Modge Podge to seal it and protect it.
If you really want to do something cool, look at Shane Speal's wood burning tutorial, print it out and get some carbon paper- get a decent wooden box- even one from a craft store like Michael's that are plain and wood burn the board image onto the box.
With either of these methods, don't be limited to putting the image square on the face of the box... you could angle it, have it fold over and continue on the sides, etc.
I'm going to be doing something similar to the first method on a box down the line....I'm collected odd post cards and I've going to cover a box with them and mod podge, tearing them, laying them on so some wrap around the box- and staining them with various things before sealing it to give it an aged look.
Got the idea when the manager of a Polish market I go to gave me a bizzarre post card of Pope John Paul II making googly eyes, or binocular eyes.... I thought, this would make for an awesome CBG image. Then I thought about building layers up like a wall that for decades has had band posters glued on and torn off.

Pinterest has hundreds of cigar box images. I made a ouija cbg by printing the image and glued it to a hardwood lid. You can use "modge podge" as Michael suggested. or simply make your own with 50/50 white glue and water. Paint this stuff onto the back of the print and a coat on the wood. The paper gets very delicate when it gets wet. Gently smooth it out and when it is dry put another couple of coat. Then you can topcoat it with whatever you like to use as a finish.
